This waiver must be completed by every individual (or parent/guardian if under 18) attending a gym rental event at Union Community Church (“Church”).

I, the undersigned, acknowledge that participation in activities at the Union Community Church gymnasium may involve physical exertion and carry inherent risks, including but not limited to bodily injury, illness, or property damage.

By signing below, I agree to the following:

1. Assumption of Risk

   I voluntarily assume all risks related to participation in activities at the Church gym, whether those risks are known or unknown.

2. Release and Waiver

   I release and hold harmless Union Community Church, its leaders, employees, volunteers, members, and affiliates from any and all liability for injury, loss, or damage arising from my presence at or use of the gym facility.

3. Conduct Agreement

   I agree to behave respectfully, follow all safety guidelines, and comply with instructions from Church staff or event supervisors. I understand that alcohol, drugs, tobacco, and disorderly conduct are strictly prohibited on Church property.

4. Medical Acknowledgment

   I certify that I am physically able to participate in the activities and will inform the event coordinator of any medical concerns.

5. Communicable Disease Acknowledgment

   I understand the risks of exposure to illnesses such as COVID-19 in public or group settings and agree to follow all posted health protocols.

If under 18, this section must be completed by a parent or legal guardian:

I certify that I am the parent or legal guardian of the above-named minor and give permission for them to participate in activities at Union Community Church. I have read and agree to the terms of this waiver on their behalf.
